Reliant traces its history to 1929, when it began as Fallon Clinic. The organization changed its name to Reliant Medical Group in 2011, merged with Southboro Medical Group in 2015 and became part of Optum in April 2018.

Identify the type of provider you need Start with family medicine, internal medicine or pediatrics when you need a regular primary care provider. Search a specialty only when you already know the department or have a referral.
Filter for providers accepting new patients The Reliant doctor search includes an option to display only providers currently accepting new patients. Availability can change between the search and registration call.
Check the location and language Search by address or distance and select a language or provider-gender preference when those factors matter to your care.
Verify your exact insurance plan Give Reliant the complete product and network name shown on your insurance card, not only the insurer’s company name.

Do not use MyChart for urgent symptoms. Reliant says messages needing attention in less than two business days should be handled by calling the physician office. Call 911 in an emergency.
