Portal login guide · Summit Health and Summit Medical Group Tennessee
Choose Your Summit Region Before Entering a Password, Paying a Bill or Messaging a Doctor
“Summit Medical Group portal” does not lead every patient to the same account. Former Summit Medical Group patients in New Jersey now use Summit Health’s regional portal, while Tennessee patients use the separate Summit Medical Group Tennessee system.
This guide helps you identify the right organization, register safely, recover access, use the mobile app, request refills, find missing results, manage appointments, pay the correct bill and request records that are not available in the portal.
REGION CHECK
WHERE DID YOU RECEIVE CARE?
Summit Health
New Jersey, New York and Connecticut
Summit Medical
East and Middle Tennessee
CORRECT PORTAL FOUND
Medical emergency: Do not send a portal message. Call 911 for chest pain, severe breathing difficulty, possible stroke, uncontrolled bleeding, unconsciousness, severe allergic reaction, major trauma or another potentially life-threatening condition.
Most common login mistake: A Tennessee Summit Medical Group account will not open Summit Health records from New Jersey, New York or Connecticut. Use the organization shown on your appointment confirmation, statement or provider profile.

Likely organization |
Correct next step |
| Summit Health, NJ, NYC or Long Island |
Summit Health |
Use the New Jersey/NYC/Long Island portal route. |
| Summit Health, Westchester or Connecticut |
Summit Health or former Westmed |
Use the Westchester/Connecticut regional portal route. |
| Summit Medical Group, Knoxville or Tennessee |
Summit Medical Group Tennessee |
Use summitmedical.com and its Tennessee portal. |
| Summit Health urology and MyChart activation |
A Summit Health urology practice |
Use the MyChart instructions from the urology office. |
| A newly joined Tennessee office mentions Athena |
Summit Medical Group Tennessee |
Follow the portal instructions on that exact office page because recently affiliated offices may have migration steps. |
Stop before paying: A familiar Summit name is not enough. Confirm the organization, patient, service date and account number before entering card or bank information.
First-time portal setup
How to create a Summit portal account correctly
Open the portal through the official organization
Use Summit Health for NJ, NY and CT care or Summit Medical Group for Tennessee care.
Select Create Account when available
Summit Health also allows patients to ask the provider’s office for an email invitation.
Use the email stored in your medical record
A different email can prevent the invitation from matching your patient identity.
Open the secure invitation
Check spam or junk folders, but verify the sender and destination before selecting a link.
Enter matching patient information
Use the legal name, date of birth and contact information the medical office has on file.
Create a unique password
Do not reuse the password from email, banking or another medical portal.
Complete registration before opening the app
Summit Health states that portal registration must be complete before the athenaPatient app will work.
Test access before an urgent need
Confirm that the correct provider, appointments and patient name appear.
Portal invitation call script
“I receive care from [provider] at [location and state]. I need access to the correct Summit patient portal. Please confirm which portal my office uses, the email address connected with my record and whether you can resend the secure invitation.”
Login troubleshooting
Summit Medical Group portal not working? Check these causes in order
You opened the wrong regional portal
Return to the organization selector and choose the state where your care occurred.
Your urologist uses MyChart
Follow the urology office’s MyChart instructions instead of repeatedly resetting the standard portal password.
The invitation email never arrived
Check spam and ask the office to confirm or correct your email before resending it.
Your email address changed
Contact the treating office. A new email entered only on a login screen may not update the medical record.
The reset link expired
Request a fresh link and use the newest email rather than opening an older reset message.
You created more than one account
Contact support rather than guessing which account contains the complete record.
The mobile app rejects the password
First confirm that registration works in a web browser, then use the same credentials in athenaPatient.
A recently joined office uses a new system
Check the exact office page or call that office because portal migration instructions can differ.
The page loads but your information is missing
Confirm the patient name, provider and organization. You may be logged into an older or unrelated record.
The portal is temporarily unavailable
Call the provider’s office for a time-sensitive appointment, refill or clinical question.
Fastest fix: Call the office where you receive care and ask staff to confirm the portal name, email on file and whether the account invitation has already been accepted.

APP
Mobile access
Eligible Summit Health users can access the portal through athenaPatient.
Portal limitation: A portal is a convenient view of selected information, not necessarily the complete legal medical chart.
Summit Health mobile access
How to use the athenaPatient app with Summit Health
Complete web registration first
The app cannot replace the initial Summit Health account-setup process.
Install athenaPatient from your device’s official app store
Verify the app name and publisher before installing.
Use the same portal email and password
Do not create a second account solely for the mobile app.
Check the provider and patient identity
Confirm that you entered the correct Summit Health portal account.
Protect phone access
Use a device passcode and biometric protection when available.
App scope: The athenaPatient instructions apply to Summit Health’s standard portal. A urology practice using MyChart or a Tennessee office using different migration instructions may require another app.
Secure communication
How to write a portal message that gets a useful response
Use one main concern per message
State when the problem started
Describe important changes or warning signs
List treatments already tried
Include the medication and pharmacy when relevant
Ask one clear next-step question
Useful portal-message template
“I saw [provider] on [date] for [condition]. Since then, [brief change]. I have tried [medicine or action]. I do not have [important emergency warning signs, when true]. Should I continue the current plan, schedule an appointment or obtain another test?”
Summit Health response timing: The organization states that it makes every effort to respond to portal messages within two business days.
Call instead when: The issue is rapidly worsening, time-sensitive, related to severe symptoms or needs an answer before the normal portal-response window.
Medical-record warning: Summit Health states that messages sent through the portal become part of the official patient record.
Appointment management
How to schedule, change or cancel a Summit appointment
Choose the fastest scheduling method for the situation
| Need |
Best starting method |
Important limitation |
| Routine follow-up |
Use the portal when the visit type is available. |
Not every provider or appointment type supports online scheduling. |
| New medical problem |
Call the provider’s office when triage or a different visit type may be needed. |
Do not book a routine slot for potentially urgent symptoms. |
| Cancel or reschedule |
Use available portal tools or call as early as possible. |
Follow the cancellation policy communicated by the office. |
| Tennessee Express Clinic |
Use the Tennessee portal’s Summit Express Clinic scheduling option. |
Confirm the selected clinic, date and visit type. |
| Emergency symptoms |
Call 911 or seek emergency care. |
Do not wait for an online appointment. |
Practical tip: Take a screenshot of the confirmed provider, address, date and time. A submitted request is not always the same as a confirmed appointment.
Laboratory and imaging results
Why a Summit test result may not appear yet
The test is still processing
Some laboratory, pathology and imaging results take longer than routine tests.
The result has not been released
The ordering provider may still need to review or interpret it.
An outside facility performed the test
Its result may require electronic transfer, faxing or manual scanning into the Summit chart.
You used the wrong Summit portal
Results from Tennessee will not appear in a Summit Health regional account and vice versa.
Your urology record is in MyChart
Check the portal used by the urology provider.
You need older records
Historical information may require a formal medical-record request.
Result follow-up script
“I completed [test] on [date] at [facility], ordered by [provider]. Is the result complete, has it reached the correct chart, and when should I expect an interpretation or next-step instruction?”
Do not diagnose yourself from a portal flag. “High,” “low” or “abnormal” may require interpretation alongside symptoms, history, medications and other results.
Prescription renewal workflow
How to request a refill through the Summit portal
Select the exact medication
Confirm the name, strength, directions and responsible provider.
Verify the pharmacy
Choose the correct street location, not only the pharmacy chain.
Submit before the medicine runs out
Routine requests may need clinical review and pharmacy processing.
Check whether follow-up is overdue
Refills may require an appointment, laboratory test or monitoring.
Call for a time-sensitive shortage
Do not send several duplicate messages when the medication will run out soon.
New medicine versus refill: A portal refill request is generally for a previously prescribed medication. A new antibiotic, controlled medicine or treatment change may require an appointment.
Profile and coverage updates
How to update insurance and personal information
Personal profile
Review the billing address, phone, email and emergency contact under the available profile settings.
Insurance information
Summit Health places insurance tools under Billing and Payments. Review current and pending plans.
Payment methods
Add an eligible method and choose a preferred default only after confirming the account.
Uploading a card does not guarantee coverage. Verify the provider, location and service with the insurer. An insurance plan marked pending may still need staff review.
Confirm the full plan and network name
Upload both sides when requested
Check the effective date
Remove outdated plans only when appropriate
Confirm primary and secondary order
Bring the physical or digital card to the visit
Pay the correct Summit account
How to review and pay a Summit Medical Group bill
Identify the organization
Determine whether the statement belongs to Summit Health or Summit Medical Group Tennessee.
Confirm the patient and service date
Do not pay only because the Summit name looks familiar.
Match the account number
Compare the statement with the account displayed inside the secure portal.
Compare the insurance explanation
Match the original charge, allowed amount, insurer payment, adjustment and patient responsibility.
Check for separate bills
A hospital, laboratory, imaging provider or outside specialist may bill separately.
Save confirmation
Keep the payment date, amount, account and confirmation number.

Complete chart access
Portal record versus formal Summit medical-record request
Use the method that matches what you actually need
| Need |
Best starting method |
Limitation |
| Recent result |
Check the portal. |
It may still be processing or awaiting release. |
| Visit summary |
Check available portal documents. |
Not every note may be displayed. |
| Complete chart |
Submit a formal medical-record request. |
The portal may provide only selected information. |
| Records for a new doctor |
Authorize direct release to the receiving office. |
Confirm the correct secure fax or electronic destination. |
| Urology records |
Use the Summit Health, former NJ Urology or MyChart route identified by the office. |
Different urology groups may use different systems. |
Choose the organization where care occurred
Do not send a Tennessee request to Summit Health or the reverse.
Identify the correct record category
Summit Health separates standard Summit Health, Westmed and former NJ Urology request paths.
Use a specific date range
Request only the records needed unless the complete chart is required.
Name the recipient accurately
Include the provider, organization and secure delivery information.
Keep the confirmation
Requests are processed in the order received, so save proof and the submission date.
Caregiver and dependent access
Do not share a portal password with a caregiver
Use the organization’s approved proxy, dependent or representative-access process. This protects the patient’s privacy and keeps each action connected with the correct user.
Ask the treating office which proxy form is required
Bring identification and legal documents when applicable
Use a separate caregiver login
Expect age-based restrictions for adolescent records
Update access when legal authority changes
Never send passwords by text or email
Parent access can change with age: State privacy rules may restrict parts of an adolescent’s record even when a parent previously had full access.
Portal security checklist
How to protect your Summit patient account
Use an official Summit website to reach the login
Create a unique password
Enable available multifactor verification
Do not save credentials on a public computer
Check the patient and account before payment
Sign out after using a shared device
Review unexpected profile changes
Report suspicious messages to the organization
Phishing warning: Do not enter login or payment information through an unexpected text or email link. Open the official organization website manually and navigate to the portal.
Local shortcut: Save the correct regional portal page as a browser bookmark only after confirming it through the provider’s official website.
Problem-solving board
What to do when the portal shows the wrong information
Your doctor is missing
Confirm that the provider belongs to the same Summit organization and region as the account.
An appointment is missing
Check whether it was booked under another office, specialty or portal.
The insurance is marked pending
Contact the office when the visit is approaching; pending review is not final verification.
The balance looks unfamiliar
Compare the patient, service date, organization and explanation of benefits before paying.
A result belongs to another date
Stop and contact the office if the patient identity or service information appears incorrect.
A portal message has no reply
Summit Health advises allowing up to two business days; call sooner for a time-sensitive concern.
The record looks incomplete
Submit a formal records request for the complete chart.
The portal link changed after an office joined Summit
Use the exact office’s current official page and migration instructions.
Portal shortcuts
Practical tips that save time and prevent wrong-account errors
01
Start with the state
Region is the fastest way to separate Summit Health from Tennessee Summit Medical Group.
02
Check urology instructions
Some Summit Health urologists use MyChart.
03
Register before using the app
athenaPatient requires a completed Summit Health portal account.
04
Call for urgency

05
Use one issue per message
A focused message is easier for the care team to route and answer.
06
Check the service date
This prevents paying another family member’s or organization’s account.
07
Keep records confirmation
Save the submission date because formal requests are processed in order.
08
Use proxy access
Do not share passwords with adult children, spouses or caregivers.
09
Bookmark only the official login
Avoid sponsored or look-alike portal pages in search results.
