Premier’s online request form and portal are for non-emergency communication.
Confirm the correct organization. This guide covers Premier Medical Group in New York’s Hudson Valley at premiermedicalhv.com—not unrelated organizations with the same name in other states or countries.
Service region
Dutchess, Orange and Ulster County areas of the Hudson Valley.
New portal
AthenaHealth became the active system on May 12, 2026.
Office hours
Appointment-request support generally Monday-Friday, 9 a.m.-5 p.m.

Start with the correct division
Which Premier Medical Group service should you contact?
PRIMARY CARE
Routine, preventive or ongoing care
Start with internal medicine or family practice for annual care, chronic-condition management, medication monitoring, common illnesses and referral decisions.
Best for: wellness care, diabetes, blood pressure, ongoing symptoms, medication questions and care coordination.
PREMIER SPECIALTY
Condition-specific evaluation
Contact the relevant specialty when you have a referral, established diagnosis, planned procedure or clear department instruction.
Examples: urology, gastroenterology, cardiology, gynecology, neurology, podiatry, dermatology and rheumatology.
911 OR EMERGENCY DEPARTMENT
Potentially life-threatening symptoms
Do not wait for an online request, portal response or routine appointment when delay could threaten life or permanent function.
Use immediately for: chest pain, severe breathing trouble, stroke signs, major injury or uncontrolled bleeding.
Practical rule: Call your Premier provider’s office for time-sensitive but non-emergency guidance. The online appointment form is intended only for non-urgent scheduling requests.
Hudson Valley multispecialty care
What Premier Medical Group provides
Premier combines primary care, medical specialties, surgical specialties, diagnostic testing and selected procedure centers across several Hudson Valley communities.
Primary and coordinated care
Internal medicine, family practice, pulmonology, nephrology and related ongoing medical care.
Medical specialties
Cardiology, dermatology, gastroenterology, gynecology, neurology, rheumatology and other focused care.
Surgical and diagnostic support
Urology, podiatry, plastic surgery, endoscopy, pathology, blood-draw centers and in-house imaging.

Provider search workflow
How to find the right Premier Medical Group doctor
Choose primary care or specialty care
Start with primary care when you do not know which specialist is appropriate.
Select the exact division
Similar symptoms can belong to different departments. Use a referring clinician’s diagnosis or recommendation when available.
Review every office listed for the provider
Premier clinicians may work in Poughkeepsie, Fishkill, New Windsor, Newburgh, Kingston or another satellite office.
Call the direct specialty number
The location directory provides department-specific telephone numbers that can reduce unnecessary transfers.
Confirm new-patient availability
A provider may appear in the directory but have limited or future availability for new patients.
Verify insurance participation
Give the office the complete carrier, product and network name shown on the card.
Ask about referrals and authorizations
Insurance may require approval even when the provider participates in the plan.
Save the complete address and suite
Several departments use the Medical Arts Building at 243 North Road but occupy different suites.
New-patient call script
“I would like to schedule with [provider or division]. My insurance is [complete plan and network]. Is the provider accepting new patients at this location? Do I need a referral or authorization, what records should I send, and what amount may be due at check-in?”

Booking and confirmation
How to request a Premier Medical Group appointment
Choose the correct division
The online form asks whether you are a new patient and which medical division you need.
Select a provider
Choose a specific clinician when known. Otherwise, call the division for help matching the reason for care.
Submit only a non-urgent request
Premier explicitly states that the online form is not intended for urgent or emergency symptoms.
Wait for office confirmation
The requested date and time are not confirmed until Premier contacts you and states that the slot is available.
Verify insurance and referral status
Confirm the exact insurance product and whether the office has received any required referral.
Arrive at least 20 minutes early
Premier requests early arrival during the athenaOne transition so identification and insurance information can be updated.
Cancel at least 48 hours ahead
A no-show fee may be applied when the office does not receive sufficient notice.
Online submission is not an appointment confirmation. Do not travel until the office provides the final date, time, location and provider.
Prevent check-in delays
What to bring to a Premier Medical Group appointment
Valid government-issued photo identification
Current primary and secondary insurance cards

Portal troubleshooting
What to check when the Premier portal is not working
You are using the old portal
Premier moved to Athena in May 2026. Use the portal currently linked on Premier’s official website.
Your identity information does not match
Ask the office to confirm the legal name, birth date, email and telephone number stored in the new system.
You created more than one account
Contact the office before entering information into another account or making duplicate payments.
An old result is missing
Historical information may not appear the same way after the electronic-record transition. Contact the ordering office or request records.
A new bill is not visible
Charges provided on or before May 11, 2026 remain in the previous billing system.
An appointment is unavailable online
Not every provider, division or visit type can be self-scheduled. Call the direct office.
A result has not appeared
The test may still be processing or waiting for provider review. Premier asks patients to allow up to two weeks for bloodwork communication.

Confirm that a Premier provider placed the order
The in-house draw centers are not unrestricted public laboratories.
Ask whether fasting is required
Follow the ordering provider’s exact instructions rather than fasting automatically.
Bring identification and insurance
New billing and record systems make accurate identification especially important.
Do not call the laboratory for interpretation
Premier directs result questions to the ordering provider’s office.
Allow up to two weeks
Contact the ordering office if you receive no result communication after that period.
Do not obtain urgent medical advice from the blood-draw center. Laboratory staff collect specimens but do not replace the ordering clinician’s interpretation or emergency evaluation.
Central radiology scheduling
Premier Medical Group imaging and preparation
Premier offers in-house X-ray, CT, ultrasound and coronary calcium scanning. All radiology appointments are scheduled through the centralized department.
X-ray
Quick imaging commonly used for bones, lungs and selected internal structures.
CT scan
Cross-sectional imaging used for detailed evaluation of many body areas.
Ultrasound
Sound-wave imaging for abdominal, pelvic, vascular, thyroid, kidney and other exams.
Calcium scan
Specialized CT imaging that measures calcium-containing plaque in coronary arteries.
Questions to ask when scheduling
Has the imaging order reached scheduling?
Does insurance require authorization?
Is fasting required?
Should I drink water before the exam?
Will contrast be used?
Do I need recent kidney-function bloodwork?
Should any medicine be adjusted?
Who will explain the final result?

Do I need a referral or authorization, and should I expect separate laboratory, pathology, imaging or facility charges?”
Important May 2026 transition
Why Premier Medical Group patients may receive two bills
Both bills can be valid. Premier changed to AthenaHealth billing on May 12, 2026. Older and newer balances cannot be combined because they belong to separate billing systems.
Which billing system and phone number to use
| Service date |
Billing system |
Payment or questions |
| On or before May 11, 2026 |
Premier’s previous billing system |
845-451-7233 or billing@premiermedicalhv.com |
| On or after May 12, 2026 |
AthenaHealth |

Check the service date first
This determines which statement, portal and billing telephone number applies.
Compare the statement with the insurance explanation
Review the submitted charge, allowed amount, insurance payment, adjustment and patient responsibility.
Do not combine separate balances
Premier states that old-system and Athena balances cannot be merged.
Question duplicate-looking charges
Confirm whether they are separate services, separate systems or a true duplicate before paying.
Use the correct portal
Athena payment applies to services provided on or after May 12, 2026.
Billing question script
“My service date was [date], and I received [one/two] Premier statements. Please confirm which billing system applies, explain the insurance payment and patient balance, and tell me which payment method I should use.”
Payment difficulty and no-show policy
Financial hardship and cancellation guidance
Unable to pay in full
Premier asks patients experiencing financial hardship to contact the billing department to discuss possible arrangements rather than ignoring the statement.
Unable to keep an appointment
Give at least 48 hours’ notice. Premier states that a no-show fee may be billed when adequate cancellation notice is not provided.
Best timing: Discuss payment difficulty before the balance becomes seriously overdue, and cancel an appointment as soon as you know you cannot attend.
Medical-record request
How to request Premier Medical Group medical records
Check Athena first
The portal may provide the specific result or visit information you need immediately.
Download the official release request
Use the Medical Records Release Request listed on Premier’s patient-forms page.
Identify the patient accurately
Use the legal name, date of birth, address and current contact information.
Choose a focused date range
Request only the period needed when a smaller record set will solve the problem.
Name the record type
Examples include visit notes, laboratory results, imaging reports, actual images, procedure reports and medication history.
Identify the recipient
Provide the complete physician, patient or organization name and approved delivery details.
Complete sensitive-information permission
Certain behavioral-health, substance-use, HIV or reproductive records may require specific authorization.
Sign with proper authority
A guardian, health-care agent or estate representative may need supporting documentation.
Confirm receipt with the office
Save a copy of the request and ask when the recipient should expect delivery.
Imaging transfer tip: A written radiology report and the actual CT, ultrasound or X-ray image files are different. Ask the receiving specialist which one is required.
After the appointment
Results, prescription refills and referral troubleshooting
You are waiting for bloodwork
Allow the published two-week period, then call the ordering provider if no communication arrives.
A result is marked abnormal
Do not diagnose yourself from a portal flag. Ask the ordering clinician how the result fits your symptoms and history.
You need a refill
Submit the request before medication runs out and confirm the drug, strength, directions and pharmacy.
The refill was denied
Ask whether you need an appointment, laboratory monitoring, insurance authorization or a new prescription.
A specialist has not contacted you
Confirm that the referral was sent, received and authorized by insurance.
Radiology cannot find the order
Contact the ordering office and verify the destination before rescheduling.
Your portal record appears incomplete
The athenaOne transition may affect how older information appears. Request a formal copy when needed.
Your symptoms are worsening
Call the treating office. Use emergency care immediately for serious warning signs.
Result follow-up script
“I completed [test] on [date], ordered by [provider]. Can you confirm whether the result is complete, whether the provider reviewed it and how I will receive the interpretation and next-step instructions?”
