Uninsured patients and patients choosing not to use insurance have the right to request a written estimate for the expected total cost of scheduled care.

Contact the physician office The official notice directs patients to request the estimate from the clinic providing the care.
Describe the complete expected service Include the visit, laboratory work, prescription drugs, equipment and likely diagnostic testing.
Ask for the estimate in writing Keep the provider, location, service description and estimated patient cost.
Ask what is not included Outside laboratories, imaging centers, specialists or unexpected treatment may create separate charges.
Save the estimate Keep the complete document until all related bills have been received and reviewed.
Compare the final bill Check the service, provider and amount—not only the total balance.
$400 dispute threshold: The official notice states that a bill at least $400 higher than the Good Faith Estimate may qualify for the patient-provider dispute process.
