Navigate Loudoun Medical Group Without Calling the Wrong Practice
Loudoun Medical Group is a physician-owned network rather than one clinic. Its practices cover primary care, pediatrics, internal medicine, surgery, imaging and dozens of specialties across Loudoun County and surrounding Northern Virginia communities.
Because each practice may control its own appointment hours, insurance contracts, new-patient availability and office policies, the fastest path is to find the exact provider first. This guide explains that process and brings the network’s portal, urgent-care, billing, forms, records and Medicare instructions into one practical page.
FIND A DOCTOR
PATIENT PORTAL
URGENT CARE
RECORDS & BILLING
Call 911 for a serious emergency. Do not wait for a patient-portal response or urgent-care opening when someone has chest pain, severe trouble breathing, major bleeding or burns, a seizure, poisoning, loss of consciousness, serious head trauma or another condition that may threaten life.
Important network rule: A policy listed by one Loudoun Medical Group practice may not apply to every LMG office. Always confirm appointment hours, insurance, portal participation, forms, refill timing and cancellation rules with the specific practice treating you.
Provider network
More than 300 physicians and practices.
Medical range
More than 39 primary, medical and surgical specialties.

Start with the right route
Which Loudoun Medical Group service should you use?
PRIMARY OR SPECIALTY CARE
Routine, preventive or ongoing care
Find a primary-care provider for checkups, common illness, chronic conditions and referrals. Search by specialty when you need focused evaluation, testing, surgery or long-term specialist care.
Best for: preventive care, pediatrics, adult medicine, cardiology, dermatology, gastroenterology, orthopedics and scheduled diagnostic care.
LMG URGENT CARE
Non-emergency care that cannot wait
LMG operates walk-in urgent-care centers in Leesburg and Sterling for minor illness, injuries and selected diagnostic needs outside normal primary-care availability.
Best for: minor fever, cough, sore throat, urinary symptoms, sprains, minor cuts, rashes, mild asthma and selected X-rays.
911 OR EMERGENCY DEPARTMENT
Potentially life-threatening condition
Urgent care cannot replace emergency stabilization. Use emergency services immediately when symptoms are severe, sudden or may cause permanent harm.
Use for: chest pain, serious breathing problems, seizures, unconsciousness, poisoning, severe bleeding or major trauma.
LMG’s recommended sequence: Call your primary-care office first, use an LMG urgent-care center when the office is unavailable and the condition is not life-threatening, and call 911 or go to the emergency department when health is at serious risk.
Physician-owned network
What Loudoun Medical Group is—and what it is not
Loudoun Medical Group began in 2000 and has expanded into one of Virginia’s largest physician-owned multi-specialty groups. Its central organization provides technology, billing, credentialing and administrative support, while individual medical practices continue to manage direct patient care.
Centralized support
Shared technology, electronic records, billing support, credentialing and practice-management services.
Practice-level care
Individual offices control their schedules, provider availability, clinical policies and many patient-service details.
Coordinated network
Participating practices can coordinate referrals and records more efficiently than unrelated offices when systems are connected.
Do not call central billing to schedule an appointment. The central office directs appointment requests to the specific provider practice listed in the official directory.
Provider-search workflow
How to find the right Loudoun Medical Group doctor
Choose the specialty before the location
Begin with the medical service needed. A nearby building may not contain the type of provider, test or treatment required.
Open the official provider search
The directory can be filtered by provider last name, practice name, specialty, language, additional notes, office location and provider gender.
Use broad filters first
Start with specialty and preferred area. Add language, gender or provider name only when the first search produces too many results.
Open the complete provider profile
Review whether the provider accepts new patients, languages spoken, education, specialty, practice website and all listed locations.
Record the exact practice name
The practice name is important because several offices may operate in the same building or medical campus.
Call the direct practice number
Ask about new-patient availability, the correct appointment type, insurance participation, referral requirements and expected wait time.
Confirm the appointment address
A provider may work at more than one site. Save the street address, building name, suite, parking instructions and direct phone number.
Provider-availability call script
“I found Dr. [name] in the Loudoun Medical Group directory for [specialty]. Is the provider accepting new patients for [condition or visit type]? Which office should I use, does that office accept my exact insurance plan, and do I need a referral or previous records?”
More than 162 locations
How to choose the correct Loudoun Medical Group location
LMG practices are concentrated in Loudoun County and extend across Northern Virginia and nearby communities. The official directory includes offices in Leesburg, Sterling, Ashburn, Dulles, Purcellville, Reston, Alexandria, Gainesville, Winchester and other regional locations.
Confirm the practice name, not only the physician name
Check the street number and suite
Verify whether the visit is office-based or hospital-based
Ask whether laboratory work occurs in the same building
Confirm the imaging or procedure location separately
Save the direct office phone number
Ask about parking, building entrance and accessibility
Allow extra travel time during Northern Virginia rush hours
Leesburg medical-campus tip: Deerfield Avenue, Sandridge Way, Cornwall Street and Riverside Parkway contain several unrelated LMG practices. A familiar building does not mean you have the correct suite.
Sterling travel tip: Route 7, Route 28 and nearby commuter traffic can add significant travel time. Recheck the address before leaving because the Sterling urgent-care center is on Benedict Drive, not at every Sterling LMG primary-care practice.
Office-specific scheduling
How to schedule a Loudoun Medical Group appointment
There is no single appointment line for every LMG practice. General inquiries and central billing cannot reliably schedule a visit for an individual physician office.
Find the practice profile
Use the provider directory to identify the direct office telephone number and practice website.
State the correct visit type
Explain whether you need a new-patient visit, preventive exam, illness visit, chronic-care follow-up, procedure, test or specialist consultation.
Provide the full insurance product
Read the network and plan name from the card rather than giving only the insurance-company name.
Ask about referral or authorization
Confirm whether your plan requires a PCP referral, medical-group authorization, prior approval or an order for testing.
Confirm forms and records
Ask which forms must be completed and what outside records should arrive before the visit.
Save the cancellation rule
Cancellation and missed-appointment policies are controlled by the individual practice and may differ across LMG.
Complete appointment script
“I am a [new/established] patient seeking [type of visit]. My insurance is [full plan and network]. Is the provider accepting patients under this plan? Do I need a referral or authorization, which forms and records should I submit, where should I park, and what is the cancellation deadline?”
Prepare before check-in
What a new Loudoun Medical Group patient should bring
Government-issued photo identification
Current primary and secondary insurance cards
Referral or authorization when required
Medication list with strength and directions
Drug, food and material allergy list
Past surgery and hospitalization history
Relevant laboratory and imaging reports
Names of current specialists
Preferred pharmacy name and location
Completed LMG and practice-specific forms
Advance directive when applicable
Copay or expected patient payment
Fast check-in tip: LMG provides a network-wide Patient Information form, but the treating practice may also require specialty-specific histories, consents or financial policies. Ask for both sets before the visit.
Practice-specific participation
How Loudoun Medical Group insurance acceptance works
Loudoun Medical Group does not publish one universal insurance list that safely applies to all 162-plus locations. A carrier accepted by one practice or provider may not be accepted by another, and participation may differ by product, employer network, service and facility.
Verify the provider
Confirm that the individual physician or clinician participates in your exact network.
Verify the location
The same provider may have different network status or billing arrangements at another site.
Verify the service
Testing, surgery, anesthesia, pathology, imaging and facility charges may be billed separately.
Read the full plan name
Use the product, network and employer-group wording printed on your card.
Call the treating practice
Ask whether the exact provider, office and service are contracted.
Call the insurer
Verify the provider by name and office address. Ask whether a referral, authorization or designated PCP is required.
Confirm every separate facility
Verify the laboratory, imaging center, surgery center, hospital, anesthesiologist and pathology service when relevant.
Save the verification details
Record the date, representative, reference number and approved provider or facility.
Insurance-verification script
“My plan is [complete plan and network]. I am seeing [provider] at [exact address] for [visit or service]. Is the provider in network at this location? Do I need a referral or prior authorization, and will any laboratory, imaging, facility or professional charge be billed separately?”
Health portal and Healow
What the Loudoun Medical Group patient portal can do

Portal troubleshooting
What to do when the LMG portal or Healow app is not working
Your practice does not appear
The office may not participate in the central LMG portal. Call the practice and ask which portal or records process it uses.
The Healow app reports an outage
LMG warns that increased demand can produce intermittent outages. Retry later or use the browser portal.
A laboratory result is missing
Ask whether the test is complete, reviewed and released. A completed test is not always immediately visible.
The medication list is incorrect
Use Message the Practice or Ask a Doctor, or call the provider office to request a clinical review.
The refill has not reached the pharmacy
Confirm the pharmacy, medication and office processing time. A portal request is not the same as an approved prescription.
You need a complete chart
Request a personal health record or submit the formal Release of Medical Records form to the practice.
You cannot see an appointment
Not every office activates all portal features. Call the direct practice number to confirm the visit.
You need proxy access
Ask the office for its authorized caregiver, parent or guardian access process rather than sharing another person’s password.

Urgent-care problem
Non-life-threatening illness or injury needing prompt evaluation, minor burns, cuts, sprains, strains or selected X-rays.
Emergency problem
Chest pain, serious breathing difficulty, severe burns or bleeding, seizures, poisoning, unconsciousness or major trauma.
Walk-in does not mean no wait. Patients are generally seen according to clinical urgency and arrival flow. Call before travel when weather, holidays or high demand may affect operations.
Central billing support
How to review and pay a Loudoun Medical Group bill
Check these items before submitting payment
Patient name matches the person treated
Statement was issued by Loudoun Medical Group
Service date matches an appointment or procedure
Provider and practice name are correct
Insurance information is current
Insurer payment and adjustment are shown
Patient responsibility matches the explanation of benefits
A separate hospital or laboratory bill is not being confused with LMG
Wait for insurance processing when applicable
Compare the insurer’s explanation of benefits with the medical statement.
Question mismatched information
Contact billing when the provider, service date, insurance payment or adjustment does not match.
Use the official payment page
Start through the LMG website rather than an unfamiliar search advertisement or unsolicited message.
Save the confirmation
Keep the payment amount, date, account information and emailed receipt until the balance updates.
Billing-question script
“I am calling about the Loudoun Medical Group statement for service date [date] from [practice or provider]. My insurance explanation of benefits shows [status or amount]. Please explain the billed charge, insurance payment, adjustment and remaining patient responsibility.”
Portal or signed authorization
How to request Loudoun Medical Group medical records
PORTAL REQUEST
Electronic access from a participating office
Use Medical Records to request a personal health record, laboratory report, diagnostic report or referral information.
Best for: convenient access when the treating practice participates and the requested information is available online.
SIGNED RELEASE FORM
Formal transfer to another recipient
Download the LMG Release of Medical Records form and identify the patient, records, date range and recipient.
Best for: transferring records to another doctor, insurer, attorney, school or the patient.
DIRECT PRACTICE CONTACT
Office does not use the central portal
Contact the practice that created and maintains the record for its fax, mailing or secure-delivery instructions.
Best for: nonparticipating offices, older records or specialty records not visible in Healow.
Identify the correct practice
Determine which physician office created the visit note, test order, procedure report or other record.
Choose the smallest useful record set
State the date range and whether you need visit notes, laboratory reports, imaging reports, referrals, billing records or a complete chart.
Complete the recipient information
Enter the full name, address, fax or secure delivery details for the person or organization receiving the records.
Sign the authorization
A parent, guardian, healthcare agent or legal representative may need documentation showing authority.
Submit to the record-holding practice
Do not assume that the central billing department controls the physician’s medical chart.
Save proof of submission
Keep the completed form and fax, upload, mailing or portal confirmation.
Medical-record request script
“I need records from [practice and provider] for dates [range] sent to [recipient]. Does your office use the LMG portal request or the signed release form? Where should I submit it, what identification is required, and when should I follow up?”
Patient-centered coordination
How referrals and coordinated care work inside LMG
LMG’s patient-centered medical-home model emphasizes access, planned care, care management, patient involvement and coordination between primary care, specialists, hospitals and other facilities.
Your primary-care team
Tracks preventive needs, chronic conditions, referrals and important outside care when information is provided.
The specialist
Receives the referral reason, relevant records and authorization, then returns recommendations to primary care.
The patient
Provides complete history, reports outside visits and confirms that records reach the correct office.
Ask whether insurance requires a referral
The medical need for a specialist and the insurance requirement for a referral are related but separate issues.
Confirm the receiving specialist
Record the practice name, provider, location, phone and fax.
Send relevant records
Include recent notes, laboratory results, imaging reports and treatment history that support the referral.
Verify authorization
Save the authorization number, approved provider, service and validity dates.
Confirm specialist receipt
Call before travel when the appointment depends on a referral or outside records.
Return the specialist plan to primary care
Make sure medication changes, diagnosis and follow-up recommendations are added to the medical-home record.
Published response expectation: LMG’s medical-home page says routine clinical-advice calls should be returned within two business days and urgent clinical-advice calls within one business day. Call 911 rather than relying on this timeframe for an emergency.

Covered preventive planning
What happens during an LMG Medicare Annual Wellness Visit?
An Annual Wellness Visit is not a complete physical. LMG explains that the visit focuses on history, health risks and a preventive plan rather than a full head-to-toe examination.
What the visit covers
Medical history, risk for disease, current health status and a personalized plan for staying well.
Measurements
The visit includes height, weight and blood-pressure measurement.
Possible next steps
The care team may recommend screenings or services that occur outside the wellness appointment.
What it normally does not include
It is not the same as a yearly physical examination
It may not include listening to the heart and lungs
It generally does not include a full body examination
Screening tests and blood work may occur separately
A current illness may require another appointment
Additional services may create a copay or deductible
When Medicare patients may receive it
Welcome to Medicare visit
Available during the first 12 months after enrollment in Medicare Part B.
Yearly wellness visits
After the first eligibility period, a wellness visit can generally be scheduled once each year.
Additional medical concerns
A separate problem-oriented service may be billed when a new or ongoing condition is evaluated during the same encounter.
Bring these items
Completed Medicare wellness packet
List of physicians and specialists
Home-health agency information
Medical-equipment supplier information
Names and locations of pharmacies
All prescription medicines
Over-the-counter medicines
Vitamins, supplements and herbal products
Medication tip: LMG asks patients to bring a bag containing everything they take, not only a typed prescription list.
Surprise-bill protection
What LMG patients should know about balance billing
The federal No Surprises Act protects many patients from unexpected out-of-network balance bills for emergency care and certain services received from an out-of-network provider at an in-network hospital or ambulatory surgery center.
Emergency protection
Emergency services generally cannot be balance billed simply because the provider was outside the plan’s network.
In-network facility protection
Selected out-of-network professional services at an in-network hospital or surgery center may also be protected.
Normal patient responsibility
The patient may still owe applicable in-network copayments, coinsurance and deductible amounts.
Compare the statement with the explanation of benefits
Identify whether the insurer treated the service as in network, out of network, denied or not yet processed.
Check every billing entity
A hospital service can create separate bills from the physician, facility, anesthesia, radiology or pathology group.
Ask why a balance remains
Contact the billing office before paying an unexpected out-of-network amount.
Keep all documents
Save the estimate, consent, insurer explanation, medical bill and all communications.
Northern Virginia patient shortcuts
Practical local tips that prevent repeat calls and wasted trips
01
Save the practice name
Several LMG offices can share a building, street or similar physician name.
02
Check the suite number
Deerfield Avenue and Sandridge Way contain multiple medical suites with different services.
03
Confirm portal participation
Not every LMG practice uses the central portal or all Healow features.
04
Call the direct office
Central billing and general inquiries do not replace the treating practice’s scheduler.
05
Plan for Route 7 and Route 28
Rush-hour and construction delays can affect travel between Leesburg, Ashburn and Sterling.
06
Verify urgent-care holidays
Regular weekend hours do not automatically apply on every holiday.
07
Verify every billing entity
Hospital, imaging, anesthesia, pathology and physician charges may arrive separately.
08
Bring medicine bottles to wellness visits
This captures over-the-counter products and supplements that may be missing from a list.
09
Keep referral reference numbers
Save the approved provider, service, dates and insurer authorization number.
Problem-solving guide
What to do when the Loudoun Medical Group process is not working
You cannot find the doctor
Search by specialty, practice or location. The provider may be listed under a full legal name, middle initial or practice group.
The listed office does not schedule that service
Ask whether the provider uses another location for procedures, imaging or specialty appointments.
The portal does not recognize the practice
Confirm whether that office participates in the central LMG portal or uses another system.
The specialist has no referral
Confirm the destination fax, authorization number and whether supporting records were sent.
The insurer says the provider is out of network
Recheck the exact office address, billing entity and insurance product.
The bill does not match insurance
Compare the service date and provider with the explanation of benefits, then call 703-443-6717.
The urgent-care wait is long
Ask whether the other LMG urgent-care center is open and suitable, but do not delay emergency care for serious symptoms.
You need time-sensitive medical advice
Call the treating practice directly. Use urgent care or emergency services according to symptom severity.
