Use the Former Grand River Medical Group Clinics After the UnityPoint Transition
Grand River Medical Group no longer operates as a separate medical-group brand. Its Dubuque-area physicians, providers, employees and clinics became part of UnityPoint Clinic on May 4, 2025.
The doctors and many familiar phone numbers remained, but the clinic names, patient portal, signs, bills and medical-record process changed. This guide explains exactly where current patients should go and how to retrieve information from care received before the transition.

GRAND RIVER MEDICAL GROUP
LEGACY CLINIC SYSTEM
UNITYPOINT CLINIC
MYUNITYPOINT + EPIC
Grand River Medical Group closed under that name in May 2025. Do not assume an old website, portal bookmark, bill-pay page or directory listing is still the correct place for current care. Current clinics are operated by UnityPoint Clinic.
The transition did not automatically cancel existing appointments. UnityPoint stated that previously scheduled Grand River Medical Group appointments would be honored and transitioned, while clinics, hours and phone numbers initially remained the same.

CURRENT CLINICAL CARE
You need your doctor, a visit or a refill
Use the current UnityPoint Clinic location, physician directory or MyUnityPoint. Former Grand River physicians now appear under UnityPoint names.
Use for: appointments, current test results, routine messages, refill requests, pre-check-in and new clinical care.
LEGACY ACCOUNT OR RECORD
You need information from before May 2025
Older records may remain in eClinicalWorks even when the most important clinical information was migrated into Epic.
Use for: missing old notes, past laboratory results, legacy images, former portal documents and old Grand River balances.
URGENT OR EMERGENCY CARE
You need help today
Check for a same-day primary-care visit, use UnityPoint urgent care for a non-life-threatening problem, or call 911 for an emergency.
Do not wait for a portal response when symptoms could threaten life, breathing, blood flow or neurologic function.
What changed in May 2025
Grand River Medical Group is now part of UnityPoint Clinic
Grand River Medical Group announced integration
The organization and UnityPoint Clinic announced their planned partnership in February 2025.
The transition was finalized
On May 4, 2025, Grand River physicians, advanced practice providers and employees became part of UnityPoint Clinic.
Clinic operations continued
Existing appointments were transferred, and patients were told that locations, hours and phone numbers would initially remain unchanged.
Patient-facing names changed
New UnityPoint Clinic names and logos began appearing on buildings, appointment messages and billing statements.
The electronic record changed
Current care moved into Epic and MyUnityPoint, while legacy Grand River records remained associated with eClinicalWorks.
What stayed familiar: Many patients continued with the same physician, provider team, office address and phone number even though the organization name changed.
What should be updated: Replace old portal bookmarks, save the new UnityPoint clinic name, check the logo on every bill and verify where pre-2025 records are stored.

Confirm the suite and department. The Delhi Street medical complex contains several clinics and independent practices. A general Delhi Street address is not enough to identify where you should check in.
Current appointment workflow
How to schedule with a former Grand River Medical Group provider
Search under the UnityPoint name
Enter the physician’s name in the UnityPoint provider directory. Do not assume the doctor disappeared because the old Grand River profile no longer opens.
Confirm the current clinic
Physicians may work at Westmark Drive, Delhi Street, Peosta, urgent-care sites or another UnityPoint location.
Check whether online scheduling is offered
Some providers display a Schedule Online option. Other appointment types still require a phone call.
Identify yourself as a former Grand River patient
This can help staff search both the current Epic chart and legacy eClinicalWorks information.
Update insurance and contact information
Bring the current insurance card and verify your mailing address, email, mobile number, pharmacy and emergency contact.
Ask whether records must be requested
The most important information may have migrated, but older notes or results may still require a legacy-record search.
Complete MyUnityPoint pre-check-in
Review demographics, allergies, medications and current health information before the visit when the option appears.
Save the current clinic name
Record the UnityPoint location name, address, suite, direct phone and arrival time.
Appointment call script
“I was previously a Grand River Medical Group patient of [provider]. I need an appointment for [reason]. Is the provider now at this UnityPoint Clinic location? Do you have my current Epic chart and older eClinicalWorks information, and is there anything I need to request before the visit?”

Portal responses are not immediate. UnityPoint says most non-urgent questions receive a response in approximately two to three business days.
Some detailed clinical messages may be billed. Messages requiring a provider to evaluate a new symptom, change medication or review a chronic condition may be submitted to insurance. UnityPoint lists a possible cost of approximately $36 to $70 for some patients with no coverage or a high deductible.
Two electronic record systems
Why older Grand River Medical Group information may be missing
Former Grand River Medical Group records were housed in eClinicalWorks, commonly shortened to eCW. UnityPoint transferred the most pertinent information into its Epic system, which powers MyUnityPoint.
Information likely to appear in MyUnityPoint
- Current medication and allergy information
- Recent problem-list information
- Current UnityPoint visits
- Results released through Epic
- Current appointment and billing activity
Information that may require a legacy search
- Older Grand River office notes
- Historical laboratory results
- Old referral documents
- Scanned forms and correspondence
- Complete historical record sets
Your doctor can see the history, but you cannot
Some migrated clinical data may be available to the care team without appearing as a downloadable portal document.
An old result is absent
Submit a formal records request identifying the test, approximate date and former Grand River location.
You need a complete chart for another provider
Ask for both current UnityPoint records and the former Grand River record range.
You still have an eCW login
Do not rely on indefinite access. Download needed information or request a formal copy while legacy records remain available.
Best request wording: “Please search both Epic and the former Grand River Medical Group eClinicalWorks record for the requested date range.”

Coverage changed with the organization
How to verify insurance after the UnityPoint transition
A plan that covered Grand River Medical Group before May 2025 should not be assumed to cover every UnityPoint physician, clinic, hospital or service in the same way.
Use the full plan name
Give the insurer, product and network shown on the card.
Verify the current physician name
Ask whether the physician is in network under UnityPoint Clinic.
Verify the exact street address
Network status can differ between clinic and hospital settings.
Describe the planned service
Coverage for an office visit does not guarantee coverage for laboratory work, imaging, allergy testing, dialysis or a procedure.
Ask about referral and authorization rules
Confirm whether the primary-care provider, clinic or insurer must submit the request.
Record the reference number
Save the insurer representative’s name, call date and reference number.
Plan-name warning: UnityPoint specifically advises that listing an insurance company does not mean every plan offered by that company is accepted.
Insurance call script
“My former Grand River provider now practices through UnityPoint Clinic. Is [provider] at [exact clinic address] in network under my [complete plan name]? Do I need a new referral or authorization because the organization changed?”

How to obtain a UnityPoint cost estimate
Ask the insurance company
The insurer may estimate the allowed amount, deductible, copay and coinsurance using the exact provider and service.

Results and medication follow-up
What to do after a test, referral or prescription request
A new result appears before the doctor calls
Wait for the clinical interpretation unless urgent instructions are shown. A result outside the reference range is not automatically an emergency.
An old result is missing
Ask whether it remains in eClinicalWorks and request the precise test and date.
The pharmacy has no refill
Check whether the medication is eligible in MyUnityPoint, confirm the pharmacy and call the current clinic when the supply is low.
The specialist has not contacted you
Confirm that the referral was sent, authorized and received by the correct current UnityPoint location.
The portal message is unanswered
Allow the normal non-urgent response period. Call the clinic when the concern is time-sensitive.
Your symptoms are worsening
Contact the clinical office for updated instructions or use urgent or emergency care according to symptom severity.
Transition troubleshooting
Common Grand River Medical Group problems and their fastest solution
The old website no longer works
Search the provider or location through UnityPoint rather than repeatedly opening the retired Grand River page.
Your doctor is missing from an old directory
Search the doctor’s name in the UnityPoint physician directory and verify the current practice location.
You cannot see old records in MyUnityPoint
Ask Health Information Management to search the former eClinicalWorks record.
You received two different bills
Compare the logos, service dates, account numbers and whether one charge is from a clinic, hospital or independent provider.
Your old balance is not in MyUnityPoint
Call the legacy Grand River balance number at 563-589-4030.
The clinic says your insurance changed
Verify the provider, location and service under the current UnityPoint organization.
A specialist lacks your history
Request both current Epic records and the former Grand River eCW record range.
Your appointment shows a new clinic name
Compare the address, physician and phone number. The name change may reflect the UnityPoint transition rather than a different doctor.
Save Your Spot time changed
Urgent-care arrival times are estimates and can move as higher-priority patients arrive.
You have urgent symptoms while waiting for a reply
Do not wait for a portal message. Call the clinic, use urgent care or call 911 depending on severity.
Local Dubuque patient tips
Practical ways to avoid repeat calls and wasted trips
01
Search the new clinic name
Old Grand River location names may lead to retired pages or incorrect portal links.
02
Save the suite number
Delhi Street contains several medical offices in the same complex.
03
Mention both record systems
Ask whether staff checked Epic and the former eClinicalWorks chart.
04
Check the bill logo
A legacy Grand River balance and a current UnityPoint bill may require different payment routes.
05
Use Save Your Spot carefully
It can reduce uncertainty but does not reserve a fixed clinical appointment time.
06
Call before weekend travel
Urgent-care capacity, holiday hours and weather closures can change.
07
Ask for actual images
A radiology report does not replace the image files when another specialist needs to review them.
08
Update proxy access
Caregivers and parents may need new MyUnityPoint proxy permissions after the system change.
09
Record every reference number
Keep insurance, billing, referral and medical-record confirmation numbers together.
Emergency and crisis care
When not to wait for a former Grand River clinic
Call 911 for a life-threatening medical emergency. Examples include severe chest pain, serious breathing difficulty, stroke symptoms, uncontrolled bleeding, a major traumatic injury, loss of consciousness or a seizure lasting longer than five minutes.
Mental-health crisis: Call or text 988 for immediate crisis support, including suicidal thoughts or concern that someone may harm themselves.
