North San Diego and Southwest Riverside · Doctors, portal, referrals and urgent care

Navigate Graybill Medical Group From Primary Care to Specialists Without Delays

Graybill Medical Group provides family medicine, internal medicine, pediatrics, urgent care and specialty services across North San Diego County and Southwest Riverside County. Its main patient systems remain branded Graybill even though the organization also works within Palomar Health Medical Group.

This guide explains where each office is located, which suite to use in Escondido, how early to arrive, how the NextGen portal works, when referrals are approved, how bills and records are handled and what to do when you need care after your regular office closes.

Name and network detail: You may see “Graybill Medical Group,” “Palomar Health Medical Group–Graybill” or Palomar Health branding. Continue using the Graybill portal and Graybill bill-pay route unless your current statement or office gives different instructions.
Call 911 for a medical emergency. Do not wait for a portal message, referral decision or office callback for chest pain, severe breathing difficulty, stroke symptoms, uncontrollable bleeding, loss of consciousness, serious trauma or another potentially life-threatening condition.
Current data-event notice: Palomar Health Medical Group has published a notice concerning unauthorized network access from April 23 through May 5, 2024. The review was finalized September 4, 2025. Patients with questions can call 1-833-931-4455, Monday-Friday, 6 a.m.-6 p.m. Pacific, excluding holidays.

Which Graybill service should you use?

PRIMARY CARE Routine or ongoing health need

Start with family medicine, internal medicine or pediatrics for preventive visits, chronic-condition management, common illnesses, medication follow-up and coordination of care.

Best for: annual exams, diabetes, blood pressure, vaccinations, medication review and non-emergency symptoms.
URGENT CARE Problem needing treatment within 24 hours

Graybill directs patients to its Escondido urgent-care clinic when a non-life-threatening illness or injury requires prompt treatment and a regular physician is unavailable.

Best for: minor injuries, flu-like illness, ear pain, urinary symptoms, minor cuts and other uncomplicated acute problems.
911 OR EMERGENCY DEPARTMENT Potentially life-threatening symptoms

Urgent care and primary care do not replace emergency treatment when delay could threaten life, an organ, a limb or permanent function.

Use immediately for: chest pain, severe breathing problems, stroke signs, major bleeding, fainting or serious trauma.
Simple decision rule: Use your PCP for planned care, Graybill Urgent Care for prompt non-emergency treatment and 911 for dangerous symptoms.

Organization explained

Graybill Medical Group and Palomar Health Medical Group

Graybill was founded in 1932 and has grown into a major primary and specialty-care group serving communities from Oceanside and Vista to Escondido, Fallbrook, Murrieta and Temecula.

Graybill and Arch Health announced an affiliation effective December 1, 2020, creating Palomar Health Medical Group. The goal was to expand access, coordinate care and provide a broader specialist network while allowing patients to continue seeing their existing physicians.

Graybill Medical Group

The name used for its provider directory, locations, NextGen portal, patient forms and separate Graybill bill-pay system.

Palomar Health Medical Group–Graybill

The network description used in current referral and case-management information.

Palomar Health

The broader health system that can be involved in hospital, emergency, specialist and coordinated-care services.

HMO patients should not assume the names are interchangeable. Confirm that the insurance company lists Graybill Medical Group or the specific Graybill physician as the assigned PCP and medical group.

Appointment workflow

How to schedule the correct Graybill appointment

Choose primary or specialty care Decide whether the concern belongs with family medicine, internal medicine, pediatrics or a specialist.
Find a current provider Filter the official directory by specialty, location, gender or language.
Check new-patient availability A provider appearing in the directory may have limited openings or may not accept new patients.
Call the correct scheduler Use 760-291-6700 for primary care. Use the direct specialist number when one is published.
Verify insurance and PCP assignment Give the office the complete plan and network. HMO patients should confirm that Graybill is assigned.
Describe the visit accurately Explain whether the appointment is preventive, problem-focused, a follow-up, a procedure or a referral consultation.
Ask about telehealth Graybill offers telehealth for selected visits. The treating office must determine whether the concern can be managed remotely.
Save both appointment times Record the arrival time and the separate provider appointment time.
Scheduling call script
“I need a [new-patient, preventive, follow-up or problem] appointment with [provider or specialty]. My insurance is [complete plan and network]. Is the provider accepting patients, is Graybill assigned as my medical group, which office and suite should I use, and what forms or records should I complete?”

Graybill asks patients to complete the registration packet at least two days before the appointment. Adult, pediatric and annual-wellness forms are available in English and Spanish.

Select the correct packet Choose adult registration, pediatric registration or the annual-wellness questionnaire.
Use the secure online form when possible The online packet is transmitted to the Patient Registration Desk.

Portal troubleshooting

What to do when the Graybill portal does not work

The account will not connect Confirm that your name, birth date, email and insurance information match the office record.
Your password or security answer fails Remember that the credentials and security answers are case-sensitive.
An appointment is missing Call the treating office and verify which patient account was attached to the appointment.
A result is not visible Ask the ordering office whether the test is complete, reviewed and released.
You cannot request a refill This is expected under Graybill’s published workflow. Contact the pharmacy instead.
Your address or email changed Update the information under portal settings and confirm the change with registration staff.
You see the wrong patient Sign out immediately and contact portal support. Do not continue opening records.
You need a complete medical record Use the formal HealthMark or signed-authorization process rather than relying only on portal downloads.
Published portal-support hours: Graybill’s NextGen user guide lists help at 760-291-6924, Monday-Friday, 8:30 a.m.-4:30 p.m. When that line is unavailable, call the main office at 760-291-6700.
After the visit

Test results, prescription refills and follow-up

Test-result timeframe

Graybill says a member of the medical team generally contacts patients with results within 3–5 business days.

Prescription-refill timeframe

Contact the pharmacy at least 3 business days before the medicine runs out. Allow 10 business days for mail-order processing and shipping.

No result after five business days Call the ordering office and ask whether the result is pending, reviewed or awaiting a provider explanation.
The portal labels a result abnormal A flag alone does not explain severity. Ask the provider how it relates to your health and symptoms.
The pharmacy has no refill approval Confirm that the pharmacy sent the request to the correct Graybill provider and office.
The medicine requires monitoring The provider may require a visit, examination or laboratory test before renewing it.
Your symptoms are worsening Call the office or use urgent care. Seek emergency treatment for serious warning signs.
A specialist needs the result Ask whether Graybill sent the report and whether the specialist also needs the original image or test data.
Result follow-up script
“I completed [test] on [date] for [provider]. It has been [number] business days. Can you confirm whether the result is complete, whether it has been reviewed, how I will receive the explanation and what follow-up is required?”
Coverage verification

Insurance accepted by Graybill Medical Group

Graybill lists many fee-for-service, HMO and PPO plans. An insurer appearing on the list does not guarantee that every product, physician, location or service is covered.

Major insurers and networks listed by Graybill
Insurer or network Coverage reminder What to verify
Anthem Blue Cross Commercial, employer and Medicare products can use different networks. Exact network, provider and PCP assignment.
Blue Shield of California HMO and PPO products have different referral rules. Medical group, hospital and specialist network.
Cigna Coverage depends on the exact employer or individual product. Provider, location and authorization.
Health Net Marketplace, commercial and Medicare products may differ. Graybill assignment and service coverage.
Humana Often associated with Medicare products, but network rules vary. PCP, medical group and specialist referral requirements.
Medicare Original Medicare differs from Medicare Advantage. Individual provider participation and secondary coverage.
UnitedHealthcare Commercial and Medicare networks can be different. Exact product and Graybill provider listing.
Other listed networks Beech Street, First Health, MultiPlan, Sharp Health Plan and TRICARE are also shown. Direct network access, authorization and patient cost.
Appointment confirmation is not coverage confirmation. Graybill specifically warns that being scheduled with a provider does not guarantee that the insurer will pay.
Insurance-verification script
“My plan is [complete product and network]. I will see [provider] at [exact address] for [visit, test or procedure]. Is the provider and facility in network, is Graybill assigned as my medical group, and do I need a referral or prior authorization?”

How to pay and review a Graybill Medical Group bill

Identify the billing organization Confirm that the statement is for Graybill Medical Group rather than Palomar Health hospital care or another PHMG practice.
Match the patient and service date Check the provider, location, date and account number.
Compare the explanation of benefits The insurer’s explanation of benefits is not a bill. Use it to review the allowed amount and patient responsibility.
Check insurer payments and adjustments Confirm that the statement reflects insurance payment, contractual reduction and previous patient payments.

Practical North County and Southwest Riverside tips

01
Save the Second Avenue suite

Several primary and specialty departments share the same Escondido address.

02
Keep both appointment times

Your arrival time is earlier than the provider appointment time.

03
Complete forms two days early

This gives registration time to review the packet before the visit.

04
Call the pharmacy for refills

The published NextGen process does not fulfill medication refill requests.

05
Count working days

Referral and records timeframes exclude weekends and holidays.

06
Verify HMO assignment

Being scheduled does not prove that the health plan recognizes Graybill as your medical group.

07
Use Graybill bill pay

Do not accidentally submit a Graybill account through the Arch/PHMG payment route.

08
Bring original imaging

Orthopedic, surgical and specialty offices may need the actual image files, not only a report.

09
Use the current Murrieta address

Older directories may show a previous Medical Center Drive location.
