CHECK-IN READY
Medical emergency: Call 911 immediately or go to the nearest emergency department for chest pain, severe breathing difficulty, possible stroke, uncontrolled bleeding, loss of consciousness, a severe allergic reaction, suicidal danger or another potentially life-threatening condition. Do not wait for a portal reply or routine office appointment.
Important Chino clinic transition—effective July 31, 2026:
CVMG has announced that its Chino office at 5562 Philadelphia Street, Suite 310, will close after July 31, 2026. Beginning in August, Dr. Brandon Tong is expected to see patients in Eastvale and Rancho Cucamonga, while Kiran Kaur is expected to work from Eastvale and North Fontana.
Previously scheduled appointments are expected to be moved automatically. Still, check the appointment location in healow or call 909-429-2864 before leaving home.
Patient ages
Primary care from infants through senior adulthood.
Continuing offices
Five locations after the Chino transition.

What are you trying to do with Cucamonga Valley Medical Group?
NEW PATIENT
Choose a doctor and establish care
Complete the new-patient form with your preferred provider, office and insurance information. A concierge team member should contact you to verify coverage and arrange the first visit.
Best route for: changing primary-care doctors, moving to the Inland Empire, selecting a pediatric provider or establishing ongoing adult care.
CURRENT PATIENT
Book, refill, check results or follow up
Call the main number for appointment, medication, referral, billing and records help. Use healow for available digital records, lab orders, vaccinations and TeleVisits.
Best route for: follow-up appointments, prescriptions, missing orders, approved referrals and routine health-record access.
SAME-DAY OR EMERGENCY
Decide how urgent the problem is
Call CVMG for a common non-emergency illness when the office is open. Use urgent care when timely evaluation is needed but the problem is not life-threatening. Call 911 for an emergency.
Do not use routine messages for chest pain, stroke symptoms, serious breathing difficulty, major bleeding or loss of consciousness.
Most efficient route: Choose the type of care, confirm the provider’s office after the Chino transition, verify the exact insurance product, prepare forms before arrival and save the direct appointment details in healow or your phone.

Inland Empire travel tip: A clinic that looks close on a regional map may require substantially more travel during weekday traffic near I-10, I-15 or CA-210. Compare the appointment time and provider availability—not only straight-line distance.
New-patient workflow
How to become a new Cucamonga Valley Medical Group patient
Identify the type of primary care you need
Decide whether the appointment is for an infant, child, adult, senior, annual examination, chronic-condition follow-up or a specific non-emergency symptom.
Choose a preferred office
Select Central Fontana, Eastvale, North Fontana, Rancho Cucamonga or Upland. Do not select Chino for care after July 31, 2026.
Choose a preferred provider when possible
Review the live provider directory for credentials, clinical interests, languages and current office assignments.
Enter the complete insurance product
Include the insurer, product, network, member ID, medical-group assignment and primary-care assignment shown on your card or member portal.
Submit the official new-patient form
CVMG says a concierge team member will contact new patients within 24 hours to answer questions, verify insurance and help schedule the first appointment.
Confirm the appointment details
Record the provider, office, suite, date, arrival time, visit type and any expected payment.
Complete forms before the visit
Download the English or Spanish packet, complete all relevant pages and place the appointment location in the subject line if emailing the forms.
Transfer important prior records
Ask the former provider to send medication history, recent laboratory results, imaging reports, immunizations, specialist notes and chronic-condition records.
New-patient call script
“I submitted the new-patient form for [patient name]. We prefer [provider or office]. My insurance is [full plan and network]. Can you confirm whether the provider accepts new patients, whether the plan requires CVMG to be assigned as the medical group or primary-care office, and which records and forms are needed before the first visit?”
Appointment planning
How current patients can schedule the correct visit
Annual physical or preventive visit
Ask whether the appointment is being scheduled as preventive care and whether separate new symptoms may create an additional diagnostic charge.
New illness or injury
Describe the symptom, when it began, severity and any warning signs so the team can choose an appropriate appointment time.
Chronic-condition follow-up
Ask whether laboratory testing should be completed before the appointment and whether home readings should be brought.
Child well visit or sports physical
Bring immunization history, school or sports forms and any family-history information requested by the provider.
Procedure request
Ask whether the service is offered in North Fontana or Upland, whether a consultation is needed and whether insurance authorization applies.
TeleVisit
Confirm that the symptom can be evaluated remotely and complete the healow connection, camera and microphone test before the appointment.
Current patients: CVMG directs existing patients to call
909-429-2864 for appointment help. Online options may vary by provider, appointment type and healow account.

Provider selection
MD, DO, NP or PA: which CVMG provider should you choose?
MD
A Doctor of Medicine who completes medical school and physician residency training.
DO
A Doctor of Osteopathic Medicine who completes physician training and may emphasize whole-person and musculoskeletal perspectives.
NP
A Nurse Practitioner with advanced nursing education who evaluates, treats and manages patients within professional scope.
PA
A Physician Assistant who evaluates and treats patients while working within a physician-led medical team.
Choose based on care fit, not only initials
Provider accepts your exact insurance product
Provider treats the patient’s age group
Office location remains convenient after July 31
Clinical interests fit the main health needs
Language and communication needs are supported
Provider is accepting new patients
Continuity tip: For diabetes, asthma, heart risk, COPD, sleep apnea or several medications, choose an office you can reach consistently. Regular follow-up is often more useful than selecting a distant provider you cannot visit reliably.

Do not stop a prescribed medicine simply because a procedure or laboratory test is coming. Ask the ordering clinician for exact instructions, especially for blood thinners, diabetes medicine, controlled medication or treatment that can cause withdrawal.

Coverage check
Insurance accepted by Cucamonga Valley Medical Group
CVMG states that it accepts PrimeCare and Optum, Inland Faculty plans involving IEHP and Molina, most PPO plans and Medicare. Location pages also refer to major HMO and PPO products.
An insurance-company name is not enough. One company may sell several HMO, PPO, Medicare, employer and exchange products. A doctor can participate in one network and not another.

A displayed logo does not prove every product from that insurer is accepted. |
Read the complete plan name
Use the front and back of the insurance card. Include the product, network, medical group and member-services number.
Call CVMG
Ask whether the specific provider and office participate in that exact plan.
Call the insurer
Give the insurer the provider, address and planned service. Ask for a confirmation or reference number.
Check referral and authorization rules
HMO, Medicare Advantage, procedure, imaging, laboratory and specialist services may have separate requirements.
Ask what is due at the visit
Confirm copay, deductible, coinsurance and whether an outside service may bill separately.
Insurance-verification script
“My plan is [full plan and network], member ID [number]. I want to see [provider] at [exact office] for [visit or procedure]. Is that provider and location in network? Does my plan require CVMG or another medical group to be assigned? Do I need a referral or authorization, and what amount should I expect to pay?”
Lab-order workflow
Where CVMG patients complete laboratory work and what to do with a lost order
CVMG’s patient FAQ states that laboratory work can be completed at PrimeCare and Optum Care. The correct laboratory may depend on the order, insurance, medical group and location.
Confirm whether the test is fasting
Ask whether food, drinks, supplements or morning medicines need special handling.
Confirm the laboratory location
Do not assume every PrimeCare or Optum site performs every ordered test.
Check the insurance network
A laboratory can have separate network status from the physician office.
Bring the order and identification
Carry the printed order or confirm that the digital order is visible and accepted.
Ask how results will be communicated
Find out whether the ordering provider will call, send a healow update or discuss the result at follow-up.
Lost order: Look for the digital copy in healow. Create a profile if you do not already have one. Call
909-429-2864 when the order is missing or inaccessible.
An “abnormal” flag is not a diagnosis. Laboratory reference ranges must be interpreted with symptoms, medications, age, medical history and the reason the test was ordered.
Specialist access
How the CVMG referral process works after approval
Primary-care evaluation
The provider documents the medical reason for specialist care and determines whether a referral is appropriate.
Insurance or medical-group review
The request may require approval depending on the patient’s HMO, medical group or service.
Referral approval
Approval normally identifies the specialist, service, number of visits or validity period.
Obtain the approved referral
Call
909-429-2864 and ask for the Referral department. CVMG says it can send the document through encrypted email or mail it to the home.
Contact the specialist
Confirm that the specialist received the referral, accepts the insurance and has the required records.
Referral follow-up script
“My provider submitted a referral for [specialty or service] on [date]. Can you confirm whether it is pending or approved, which specialist was authorized, the expiration date, number of visits and whether the referral can be sent to me by encrypted email?”
Save these details: Referral number, specialist name, specialist phone, approved service, number of visits, effective date and expiration date.
Medication planning
Request CVMG prescription refills before the medication runs out
CVMG asks patients to submit refill requests five to seven days before the final supply is expected to run out. Call 909-429-2864 and ask for the Medications department.
Medication name and strength
Exact directions and daily frequency
Number of doses remaining
Correct pharmacy name and address
Prescribing CVMG provider
Any required follow-up or monitoring test
The pharmacy sent a request but heard nothing
Call the Medications department and confirm the request was received under the correct patient and provider.
The medication requires an appointment
Ask what examination or monitoring is overdue and schedule before the supply is exhausted.
The insurer requires prior authorization
Ask whether the insurer sent its requirements and whether an alternative medicine is being considered.
The medication is controlled
Follow the controlled-medication agreement and do not assume early replacement, pharmacy changes or outside prescriptions are permitted.
Do not double a missed dose or stop a long-term prescription without medical advice. Contact the pharmacy, CVMG or emergency services depending on the medicine and severity of the problem.
Medical-record request
How to transfer or obtain Cucamonga Valley Medical Group records
Download the authorization form
Use the Authorization to Release or Receive Medical Records form published on the CVMG forms page.
Identify the patient clearly
Use the full legal name, date of birth, current contact information and any previous name used in the medical chart.
Name the organization sending records
Include the complete physician, clinic, hospital or facility name and contact details.
Name the recipient
State whether the records should be sent to CVMG, another provider or the patient.
Use a clear date range
Specify the dates and record types needed instead of requesting an unnecessarily large chart.
Sign and date the authorization
Missing signatures, representative authority or required consent can stop processing.
Mail the form to North Fontana
Send the completed request to 16465 Sierra Lakes Parkway, Suite 300, Fontana, CA 92336.
Allow five to seven business days
CVMG says its Medical Records representative may call when additional information is needed.
Request records early. Five to seven business days may not include mailing time, weekends, holidays, an incomplete authorization or time needed by another provider to receive and review the chart.
Statement review
What to do when you receive a Cucamonga Valley Medical Group bill
CVMG instructs patients whose statement has a return address of 299 West Foothill Boulevard to call 909-429-2864 and ask for the Billing department.
Confirm the billing organization
Read the provider name and return address. An outside laboratory, imaging center or specialist may send a separate bill.
Match the service date
Compare the statement with the visit, procedure or test you received.
Read the insurer’s explanation of benefits
Check the billed charge, allowed amount, insurer payment, adjustment and patient responsibility.
Check whether the claim was denied
A missing referral, incorrect member information or coding question may require correction rather than immediate payment.
Call before paying a disputed amount
Ask the Billing department to explain each charge and whether the claim can be corrected or reprocessed.
Billing-question script
“I received a CVMG statement for service date [date]. The statement shows [amount], while my insurer’s explanation of benefits shows [status or amount]. Please explain the charge, insurance adjustment and patient responsibility, and tell me whether any information or authorization needs correction.”
Outside-service warning: A laboratory, imaging center, hospital or specialist may have separate network status and separate billing. Confirm each outside organization before non-emergency care.
Selected office procedures
CVMG procedure clinics in North Fontana and Upland
CVMG has described procedure clinics at its North Fontana and Upland offices. The published program is intended to complete selected procedures in a familiar primary-care setting without a separate specialist consultation for some services.
Skin care
- Skin-cancer screenings
- Biopsies
- Selected excisions
- Patient education
Musculoskeletal procedures
- Selected shoulder injections
- Selected knee injections
- Trigger-point injections
- Assessment before treatment
Other published procedures
- Toenail removal
- Selected catheter removal
- IUD removal
- PICC-line removal
- Simple laceration repair
Call before scheduling. Procedure availability, provider qualifications, medical suitability and insurance requirements can change. “No referral” from the clinic does not necessarily mean an insurer will cover every procedure without authorization.
Procedure-clinic script
“I need evaluation for [procedure or concern]. Is this currently performed at North Fontana or Upland? Do I need a consultation first? Does my insurance require authorization, and are there medicines, transportation or aftercare instructions I should know before scheduling?”
Children and teens
How to prepare for pediatric, school and sports visits
B
Infant and well-child care
Bring birth history, feeding concerns, growth questions, medication doses and vaccine records.
S
School physical
Bring the school form, immunization requirements, prior records and any learning or health accommodations.
⚽

Do not wait until the school deadline
Schedule several weeks before forms are due
Bring every page requiring a clinician signature
Confirm whether vaccines are needed before clearance

Report fainting, chest pain or breathing problems
Keep a completed copy for school and family records
Flu vaccine timing: CVMG says flu vaccine usually becomes available around mid-September. Patients can schedule after the practice confirms that doses have arrived.
After the appointment

Begin five to seven days before the current supply is expected to end.
Return sooner when symptoms worsen
Follow the provider’s warning-sign instructions and use urgent or emergency care when required.
Result follow-up script
“I completed [test] on [date] for [provider]. I was told to expect the result by [timeframe]. Can you confirm whether it has arrived, whether the provider reviewed it and how I should receive the interpretation and next-step plan?”

The specialist says no referral arrived

Local patient shortcuts
Practical CVMG tips that can prevent delays and repeat visits
01
Put the office in the email subject
CVMG specifically asks patients to identify the appointment location when emailing forms.
02
Save the exact suite
Several clinics are in multi-office medical buildings, so the street address alone is not enough.
03
Check Chino appointments twice
Verify the moved provider and office before any appointment scheduled in August 2026 or later.
04
Request refills one week early
The official recommendation is five to seven days before the medication supply ends.
05
Keep referral numbers
An approval number and expiration date can save several calls when the specialist cannot locate the referral.
06
Check the lab network separately
The physician office and laboratory may not share the same insurance participation.
07
Bring outside records yourself
Carry important recent results when there is any doubt that the former office completed the transfer.
08
Prepare TeleVisit readings
Record temperature, blood pressure, glucose, weight or pulse-oximeter readings when relevant.
09
Separate preventive and new concerns
Ask how discussing a new medical problem during a physical may affect billing and appointment time.
